Uncontrollable Bleeding from the Tracheostomy Stoma in patient of Tuberculous Peritonitis


Abstract
An 8 year old female patient was admitted to our Dept. because of impatient respiratory difficulty, coughing, cyanosis and abdominal distention. During the course of admission, she was performed tracheostomy and intensive antibiotics and sulfonamide therapy due to extensive pneumonia on both lung field.
The pneumonia was marked improved,. during the hospital day, but there as no. improved the abdominal distention. Thirteen day after the tracheostomy sadden uncontrollable bleeding was noted from the tracheostomy stoma and the patient died in spite of blood transfusion.
On autopsy the: bleeding point was found at the left bracheocephalic vein as a necrotic perforation measuring 1 X I mm ,and the vein lifted up to the level of the suprasternal notch, At the opening of the abdomen, serous ascitic fluid of about 2500 ml was evacuated and the mesenterium showed the typical pattern of tuberculous peritonitis.
In the peritoneal cavity huge tumor mass was found which was identified as a well differentiated lymphoma.
